This is a form where you can record all the necessary information for when you pass away, such as, where you would like your service to be held, or what flowers your would like, which newspapers shall be contacted for obituaries, etc.


The Department of Internal Affairs require your parent’s full names, your date of marriage, your children’s current ages. Having this all recorded helps make the process easier for your loved ones who have been left behind.


After completing the form it is a good idea to keep it in a safe place with your personal documents and be sure to tell at least two others where to locate it. Alternatively, if you prefer, we can store the information for you until it is required. We strongly recommend you tell someone close that we are holding the information for you so that they know where to find it when that day comes.



You can prepay any amount into our FDANZ Funeral Trust. The maximum amount that is exempted from asset testing by the Government is $10,000.00 but it does not mean you have to use all this money for a funeral. Whatever is not used can be added to your partners trust or go back directly into your estate. It is paid out quickly and efficiently.
